I'm looking to have a simple 2 page form developed for a new website...
The form is meant to collect inquiries submitted online and store them in a MySQL Database...
A host of free and paid scripts are available to do the job on hotscripts and scriptsearch - customizing one of those is acceptable
The basic features required:
For the form:
- Validation of form elements (valid email address, dates, numbers etc)
- A calendar pop-up for date entry (Orbitz format/knock-off would be good)
- Preview before sending feature
- Ability to move back and forth between form pages without loss of data
- Storage in a MySQL database
- Send automated email on submission of form - to at least 2 email addresses (admin and submitter)
Backend database:
- Ability to browse and download data from database through password protected interface (extremely simple interface nothing fancy)

This is for a brand new site being developed... coders delivering satisfactory results are assured of multiple projects almost immediately
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request.
3) Exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

The entire site is built using static pages... form processor script should be able to be integratable with plain HTML files with .htm or .shtml exensions...

The first step/page of the form definitely needs to be a static html file with. The submission data will have to be passed to a php or perl script that can then present the second page (doesn't matter what extension the second page has) for input of further information... the final review and confirm page can also be any format.. php etc..
