Email Template Generator - Very Simple

I'm looking for someone who can design a simple product which will generate email templates. An example email is below: ================================================= Hi, [Company] are currently looking for a [Job Title] to [Company Text]. [URL] [Company Profile] [About us] [Signoff Text] With thanks, [Signature] =================================================== The way this will be generated is the following: [Company] will be a text box, as you type the text box it will be giving suggestions in regards to the company names stored within the database. [Job Title] will be a text box [Company Text] will be pulled from the database based on the [Company] [URL] will be a text box in which a number is entered, this will be used to generate a URL. [Company Profile] will be pulled from the database based on the [Company] [About us] will be pulled from the database [Signoff Text] will be pulled from the database [Signature] will be a text box, as you type the text box will be giving suggestions in regards to the signature name stored within the database. It will output specific text based on the input. Once the form has been filled out and submitted a new page will be generated with the parsed information, which will then be copy and pasted into an email client. It must copy into the email client correctly. The database will need to store the following: Company - Company Name - - Company Text - - Company Profile About us - About us Text - Signoff Text Signatures - Staff Member Name - - Signature Text Additional Companies and Signatures will need to be able to be easily created.
