PHP form with MySQL database

I created a very simple database and need to complete coding a multi-step form which uses PHP to get data from the db. While filling out the form, the script should store the entered data into session variables and ultimately display the submitted form data on screen as specifically formatted HTML file. I would also like to be able to save/export as PDF file upon completion. I would like to complete this project in the following phases: FIRST: Finish coding the form - mostly this consists of setting up variables which can be filled from a web form. Then either passing or storing the submitted data (in a session variable?) and amending to it each step. The form will have 5 to 10 steps to complete before submission. 2. Create second database table for "menu" - For one step of the form, I would like to be able to load the "menu" for the day from a second table in MySQL database which will need to be created. 3. Display submitted data on-screen as HTML 4. Export HTML as downloadable PDF file (do not store any new data on server at this time) 5. Use PHP to send an email to administrator with a notification that export is completed
