Hey, I started my own business in germany and we are selling supplements for pets. Since the supplements are personalized, i want the customer to "register" his pet. I want them to answer a quiz, name, weight, how much the pet is outside a day and so on. In the End the quiz should recommend one of our 4 Mixes combined with one of our 3 oils. I have Uploaded some pictures, what i am looking for. I am sorry the pics are on german, but its the best quiz i found and i think the picture is explaining enough without use of language.
