Database of auditable entities

I work in the audit sector. I require a database of audit entities (one particular business eg Fordham School, Kings Mart). The database can be in any language,format but needs to be simple to use for end users. A user should be able to able to add an entity to the database. The properties of the audit entity will be nature of business (once added will be saved and available for use later on for any other entity) audit period ([login to view URL]) audit observations (each entity may have any number of observations, can be added and edited as preferred) internal reference number for system for each objection. for each observation, there should be an option to update it, so future compliance can be added to the observation, without changing the original.(appending compliance and answers to the observation) observations will be rich text format paragraphs,tables,bullets the entire data should be capable of being exported to csv format.
