Microsoft Word application form - check for problems and revise if necessary

One of the young students in our office has made an application form in Microsoft Word. It is to be used by a charity, by people applying for funds. So any user who downloads the form in any version of Word should be able to open it, fill it in and save it, to be sent off to the charity for consideration. I want an expert with Micrososft Word to look over the form, check it in older versions of Word, and make any changes necessary to make it good. Macro enabling must not be required. There are mandatory fields which are clearly marked. We already have a good web-based form that does the job - this Microsoft Word version is for users who find difficulty in doing it online! So it must work perfectly and not be challenging in any way. Our first version of this in Microsoft Word showed the words "click to fill in here" in some versions of Word, which is considered unwanted. Just blank fields should show in the form waiting to be filled in. I have grey boxes at the moment and would prefer plain white blanks. I think this task would take a Word expert a very short time, so I have only allowed a small budget.
