ActiveX/.net add on for Word - secure image insertion

The project consists of a <required> portion and an <Optional> portion. We may elect to only pursue the required portion or both the required and optional portion, so please include a quote for both. <required> Add on for MS Word, presented to end-user as a toolbar button, that, when clicked, presents the user with a list of images to insert (list based on logged in user's network user name), and requiring a password before inserting the image and a time/date stamp. <Optional> When image is inserted, information is to be sent to mySQL database including the following fields: <UserName> Network Name of user inserting the image <Date_Time> Date and time image was inserted <Document> Name of document image was inserted into <Image> Name of image inserted This is an urgent project.
