Database Programing

Job: Writing simple online software Functions: I want people to be able to come to my website and enter their information into these fields (like Name, Address, Date of Birth etc.). Once they enter this information I want them to have a drop down button where they can select a field of different catergories. Once this information is entered and selected once they click submit I want the information to be put into a letter that we will store on our server and they can print it on there computer. Or it can be emailed to them if it is better.
