Add Custom Fields To Quote Request For Customer To Fill Out

I have a custom Magento site that has Cart2Quote (Standard version) installed. Cart2Quote extension knowledge is neccessary. We are looking to add 6 total fields. I have attached a demo mockup of what we are looking for. You can see that both, "Do you have a general remark with your quote request?” & “Additional Information” text areas are visible. This will not be the case when finished. We will be using the, "Do you have a general remark with your quote request?” field as the “Additional Information” field as the end result. It is just shown as a duplicate within the attached image for now. So these are the fields: Job Title: text field Job Site Address: text area field Rental Start Date: text field with magento javascript calendar input for date Rental End Date: text field with magento javascript calendar input for date Delivery Method: drop down field with 4 options 1. Local Pickup 2. Local Drop Off 3. Local Drop Off & Setup 4. Shipped Preferred Method of Contact: drop down field with 2 options 1. Telephone 2. Email I would like these fields to also be editable within the admin and viewable within all email templates and fronted templates.
