Wordpress Plugin For Quize And Gives Certificate

I'm using wordpress for my membership training and I'm would like someone to build a plugin that allows me to create a quiz with multiple choice questions, score the correct answers, and when they have scored over a certain % they would get access to a Certificate. The certificate would be a high resolution image that we would upload. The quize would start out by the user entering in thier information. First last name, company, and any extras info we need. Then when the user reaches above a % the plugin would merge the high resotion photo with the data entered and give them a .pdf of thier certificate they could print out. There are lots of free quize plugins now, so the quize side should be easy.
