magento collect data during 'add to cart'

we have magento ce 1.7. our magento instance is running 4 shops and we want this function on 1 of them. the shop sells pet clothing the idea is to collect information about the pet during the buying process : name* length* neck* chest* girth* d.o.b. this data is then added to the product line in the sales order and visable via the backend (sales->orders->view) it would be ideal to store this data in the customer account and view it in a backend grid customers->customer owns. grid: name-length-neck-chest-girth-dob-customer name-customer email. no changes to core files other shops on the magento instance not to be effected
