We are needing a database to break down demographics - the script should work as follows: 1.) We enter key information about an individual based on a survey or form they complete. Below are the common entries we would make but would like the option of entering more fields as needed for data entry. - name, address, city, state, zipcode - telephone number, cell number, email address - date of birth, sex 2.) We are able to break down that information depending upon our needs and output accordingly. For example if we just want to compile and output the average age of the respondent - it would be preferable that we could do so by gathering that info from our data and outputing it into both a chart/graph and text output. 3.) The ability to archive and print data.
