PHP module used to define and modify the records in a table

(MAGPPD) We need a PHP GUI of approx 4 pages to modify/insert records to a table by applying some comparison [login to view URL] application has also to print the elements of table that are on display. All the other constraints and the db (MySQL) structure are in the attached document. As this is part of a bigger project, PHP coding, use of CSS is a must. The test document will be provided to help the coder to submit his work. The db access, and an ftp to test the code will be provided to the coder. The final code has to work on our ftp. The program will work in Turkish therefore windows-1254 or 8859-9 sets are to be supported
