Wordpress Pop-up Form

I have a wordpress blog, and I need to add a pop-up form and with submission confirmation. 1) The first form will be a button that will be placed on the top right of the navigation. When a user clicks on the button, a form will popup and allow the user to complete and send. Similar to "Get A Lawyer Today" button on: [login to view URL] Here is another similar header button with form ("Call me back"): [login to view URL] Necessary Requirements: - Form and fields must be editable via Contact Form 7. Plugin is already installed. - Styling, CSS, etc. must be completed as part of the form (similar to samples above). - A "Thank You" prompt in the popup once form is submitted. - Data sent must be secure - A copy of the data should be saved in case there is a problem with email submission *MY BUDGET IS $40 FOR THIS FORM.
