Building a report to fax to doctors from my pharmacy

Hi There. I run a pharmacy and as part of what we do, we have standing orders with physicians who send in prescriptions for [login to view URL] pain creams or scar creams. With our standing order, I am allowed to change the cream to a different medication (also adding other creams) as allowed in our standing order agreement signed by the pharmacy and the physician. Physicians however, have asked that we report to them on a patient by patient basis at the end of the week, the name of the original cream and the name,strength, quantity, directions of the creams we substituted to and also any that we add on. I can generate a report of all the prescriptions I've done which includes patients names and their drug information in excel format easy enough. What I want to do is to create a program or macro that will sort through the data and for each patient , organize it into a word document report that will have one page for multiple prescriptions for one patient. Example Patient Dummy Person has had a cream for ABC PAIN CREAM and XYZ SCAR CREAM sent in. Based on standing orders, I interchange it to DEF PAIN CREAM AND GHI PAIN CREAM AND UVW SCAR CREAM AND PTS SCAR SCREAM. Now I want to have all this in a one page report for that patient (one page so that the nurse can easily stick it in the patients chart) including information about the original cream. The report will say something to the effect of . DEAR DR (SO AND SO) , you sent it two rx's for PATIENT DUMMY PERSON, ABC PAIN CREAM AND XYZ SCARE CREAM based on our interchange program, these rx's were substituted to DEF GHI PST AND UVW. Please keep this as a copy for your records and sign here and fax back to blah blah blah for our records. Thank you
