Facebook Form Gate for OpenCart 1.5.6.4 up to $80

We require a Form Gate for Facebook See #3 on this page [login to view URL] Ideally we would like to query the facebook API and see if the user is already in our opencart database, if the user is in our database take them to a custom html / php page If they are not in our database show them a form page where they can enter their information (hopefully some of this would be filled out based on their ID) <custom Message and or image> FirstName LastName EmailAddress MobileNumber NewsletterEnabled SMSEnabled This would have to work on the facebook app as well as the desktop / web browser application.
