Iframe and widget Form as simple API for sites - PHP/MYSQL

Hi We need a smart programmer who can create a special short request form that any web site can integrate with their site form is residing on our end but web site can fully display it on their end Iframe/or Widget Form include: about 6- 8 fields text fields about 3-4 fields optional file Uploads integration of Docusign of 1 agreement signed by us and client who fill form will sign only( if one of the optional fields is uploaded!!) you need to integrate that Customer get a link to customize his form Add his site name or logo or logo optional He can also customize text instead of banner of top of form and he can modify the text we have above the form( 4-6 lines of text) after form submit it pushes the info to our CMS / My sql DB we will give you access to create table and integrate with our admin incoming lead section to identify source Also on the uRL to customize the form the client can set his username/email and he would automatically added as a rep after he create an account under admin/ also easy integration with our Admin reps Allow for a few columns ( up to 6) added , etc So at the end of this project we should be able to send email to any site to come and login and customise their form or use default setting they can easily plug in their site as a widget or an iframe ( they can also use a selection from 4-6 banners to download and add to their site before user arrive to form/widge after clicking on banner..) and every user that fill up Send them an email ( allow in the customization area under profile) and also push info to our DB under Admin-Incoming leads and push the right fields with Source/username and site name.. and also send us an email automatically Should be simple You should allow form to be in 3 sizes, and as widget and as an iframe customization area : My Site Profile Customize Form/Widget Download Banners/Icons Logout Budget $100- $250 We will give you all forms fields and our docusign account for integration..!
