RMA system - Questionnaries generates answer in a combined text.

To avoid using too much time e-mailing and phone - then I need a system where the users are guided with a set of questions before they can create a case valid case. Within these questions we can also have images or videos to explain details better. Check the video for example of how it would be done: [login to view URL] Questions would start like this 1: Enter your name 2: Enter your E-mail 3: Enter your your phone number 4: Enter your the of product you want support for 5: Enter your invoice ID 6: Now he/she is met with multiple questions of what the problem is: 6,1: I want to pay for a fix 6,2: There is a defect on the product 6,3: I want to return the product for a refund. ...Overall, then depending on which field they press, then they will get either more questions or a piece of information follows by either a question of choices. In the end they can create a "case" or they are informed that this is not covered under warranty (Like a battery which has been mistreated) In the end we will have a filled formular with all the entered data for easier processing of a RMA case. Some of the entered data will not amount in a case. I need a system, a script or similar - could be like in the attached video.
