Drupal 7 Web-forms (Themed)

I am developing my website and need a few of the finals thing done and set up correctly so I can finish it as soon as possible. It is currently on a stage URL until we are ready to migrate from old site to new site Staging URL is [login to view URL] For an experienced Drupal Website developer the things I need done are not overly complicated just a little beyond my own personal knowledge of drupal. Membership Form in three sections (Personal Details, Surgery Details/Medical and upload Images) - create login ID and Password or option to login using Facebook - Specific field to be advised -Ability to upload Images after asking if they wish to upload now -Themed same as website -Progress Bar at top through the sections - Issued a unique Code - Upon Submit for details to be made into a PDF and Emailed [login to view URL] Booking Form -using Unique code from membership form form details to be populated into new form or referred back to create membership ID -Confirm or edit details populated from membership form -Progress Bar -Additional Surgery details Required -Select Surgeon -Additional Medical Details -Request Travel Details - Ability to upload Photos -Payment Section to accept Credit card or Paypal ability to pre-set price Submit and Email Copy [login to view URL] Booking Form Similar to Booking form requirements with additional field to choose which Group 4. Dental Only Booking Similar to booking form without medical details (all booking forms to have ability to populate information from the original Membership Form) Payment to be taken for the three booking forms The forms will Link to [login to view URL] Edit Contact block to add extra fields - Will inform on field required Move Contact Block to be inline with Office Hours ([login to view URL]) Content Type - To enable me to upload PDF for Visitors to Download I have attempted to create one but I want an image I upload to be associated with the pdf so they click the image. Currently have the image but the file name underneath I also would like to be able to add files that can be download to the basic page content type that if a file is uploaded through the edit page it will display a image file "Download Now - Detailed Information Sheet" I can create the image file to be used if needed We currently have a Menu Item Procedures ([login to view URL]) This take to main page with Sub Menus I need to clone this page set up to be a content type called "Sub Menu" that I can then create other pages set up the same as this for other [login to view URL] will create the look of this page and then to have a Content type called Sub Item which is the where I add the item to display onto the Sub Menu Page as it does on [login to view URL](link broke when I changed url name). With the images as well as the teaser on the Sub Menu Page and then a More Info button to take me to the item clicked which with be the Sub Menu Item. It does not need to have the ability as it does now on the the image to zoom but need to be able to have an image that will appear as it does now. Originally The procedure page was a portfolio [login to view URL] if I add a portfolio item it adds it to main Procedure page not on to the correct Sub Menu. even if listed it under the parent menu of body surgery but it appeared under the main page "procedures" In short I need to be able to make a Sub Menu Page and then add sub Menu Items easily through content type
