Application Page Design

We are creating a very simple search portal that will require customer login for access. There will also be a minimal administrator section. We need a graphic designer to create the design for the pages that will fit with the company's branding which is basically grayscale. We're looking for someone that can create a good user interface layout with some nice graphics. This is not a large job, but it must look very clean and professional. Please provide samples of previous work that is applicable with your bid. See the attached document for specific requirements. We will need a quick turnaround for the main search form and search results pages to get approval. Other pages will need to follow pretty quickly as well. Although there is repetition in pages so there are really only about 9 pages including simple pages like login and forgot login. So this should be a pretty quick job overall. Thanks for your interest.
