need file upload capability on apache

I have an apache web server and I have users that will be posting files via curl from a remote server. I want to build a page that my customers can access to upload csv files to be stored on the apache server. It can be very simple, but must be able to handle uploads of large csv files. I want input items for the file upload to be username, password, app, and jobname. After the file is uploaded I want the input items to be stored in a text file in the same place the file is uploaded. I will need instructions if any packages need to be installed on my apache centos linux server.
