MS Word and Visual Basic Document Project

Please provide a sample of previous work for bid consideration. Also read the information below before bidding on this project. I have an 8 document project for a total of 64 pages. I need a 64 page document converted from paper to electronic. Objectives: 1. Most of the document will require form fields. Most of the form fields will be text boxes, check boxes - in some areas allow for multiple selections, radio buttons 2. Retain the layout as close to original as possible. The areas in table format should remain as such. The areas with special formating will retain that also. 3. Give the end user the ability to update the header and footer information on opening the document. Allow the enuser to add information to the header and footer sections such as dates (From and To), names, initials. Would like to have the document be auto saved with footer information on exit. 4. Leave the document open for future edits, that is no proprietary lock. Will need to be able to make changes to the headers and content as updates are available.
