Yikes! What a challenge! I won’t tell you that I can do this in a day, or practically for free. That’s not realistic. What I can tell you, is that I regularly have to decipher medical notes (mostly hospital records) and interview notes (by 2 psychologists) for medico-legal reports. It takes time, but it can be done.
